Use this form to order your distinctive name badge.  This is for current game directors, current members of the Board of Directors, and other volunteers.  Your badge will be 1.5 X 3 inches and will display your TITLE on line 3 below your name.  Game directors will not be charged for your badge.  Board members and others will pay $12 on receipt their badge, expected in approximately two weeks.
