Hand 5: Stopping Defenders Discards

North's most damaging lead is a Spade. It sounds from the bidding that Declarer may rely on a cross-ruff and that turns out to be the case.

Declarer should let the lead run to the ♠ Q

Then play ♣ A, K, and a Club ruff. Now a Diamond ruff back and then a Heart to the  A in order to ruff a Diamond.

It's usually important to cash side-suit aces before cross-ruffing and the Heart here is important. If you ruff a Club to get back to Dummy North will throw a Diamond and you don't want that
