Enter your First Name and Surname in the same format as is held on the SBU database Mempad.  If you are usually known by another name please add this in the comments field  (eg Rob vs Robert etc)
 

We require an email address to complete this form.  In the unlikely circumstances that you do not have an email address please input "No email address" in this field.  If you do have an email address but are reluctant to share it please still input it .   When your data is uploaded you will receive a Welcome Email confirming your membership, and this details how to set restrictions regarding the use and display of your email address should you wish to do so via the Account section in the Member Area of the website.
 

You can set your Contact phone number to mobile or landline number - or both.
 

You are given an SBU number when you join the SBU.  You must be a member of the SBU to join our club.  See the SBU website for information on how to join If you are not currently an SBU member.
 

Please take particular care when entering your BBO username.  This is used to verify you are a member when joining some of our tournaments and if incorrect may block you entrance.
 

Home Club is the bridge club through which you pay your SBU levy.  If you pay your SBU levy direct to the SBU, enter the home club as "Direct ".
 

You must agree to the membership terms to ensure we can meet out Data Privacy obligations.
 

In comments you can include any other information that you think is relevant to your application.  In particular include the name you may be known by if this differs from the name held by the SBU (eg Rob vs Robert etc).
