3C by North = cue-bid of opponent's suit agreeing hearts with good support
3S by South = cue bid showing a control
4C by North = cue bid showing a control

West surely must have AD and KC for his 2NT bid. After winning the opening lead & drawing trumps (West having 2, therefore 1-2-5-5 shape), the plan should be a strip squeeze & end-play.
Your next lead is a Morton's Fork play of the 2D from hand. If West wins this trick, the hand is over - cash the KD & the club loser in South's hand disappears on North's QD. Therefore West ducks & North's queen wins the trick. The next play is to cash all the spade & heart winners to reach this position as the last heart is about to be played:
