This Form should be used to request a ruling in respect of any dispute arising during a match which cannot be resolved at the time.      Both Captains should have a copy of this Form available at the match in case it is needed.     After being fully completed, the Form should be sent to the Appeals Secretary who will appoint a referee.    A copy should also be sent to the League Secretary.       Should either Captain wish to appeal against a referee's ruling, the same Form, together with £20 (refundable when your appeal is judged not to be frivolous), will be used for the appeal.      Ask a 'Referee' if you are not sure.
